• Annual routine physicals, regardless of medical condition or necessity, for both men and women
(including gynecologic exams with PAP smears)
• Annual physical-based patient education and health guidance with immediate electronic communication connection with our practice
• Integrative wellness visits to explore root causes and solutions to avoiding health risks (all ages)
Plan-Covered Care: Chronic Care and Sickness
• Chronic Care - for diabetes, high blood pressure, high cholesterol, heart disease, asthma, COPD, arthritis, obesity, depression, anxiety, ADHD
• Acute visits for colds, sore throats, ear infections, simple injuries, skin issues, infections, STD testing, urinary tract infections,
• Skin biopsies and cryotherapy
• EKG's, flu shots, all medically necessary primary needs met per plan protocols

Annual Physical-Based Health Guidance Includes Remote Communication & Care
• Home visits for those who are homebound
• Telemedicine - appointments over the computer as needed for patients that can not get to the office or for a brief follow up that does not require a detailed exam
